Please read this first! This form is very different from the printed version. 1. Each form submission is for one sire; you only have to enter the sire information once on each form. You can register ten head on each form. 2. Each form submission is for only one type of dam ownership, i.e., either (1)owned by the breeder or (2) owned by someone else at conception or lambing. It is not possible to mix the two groups on a single form, although this may rarely be a problem because of the limitation to one sire per form. 3. You obviously can't sign this online form. We already accept your word as truthful in submitting offspring data. By entering your e-mail address in the signature space for breeding certificate, we will accept that as a declaration that you have a breeding certificate in your possession. Should there be a dispute as to the paternity of an animal, you must be able to present that breeding certificate. 4. You must have a credit on your account at least equal to the amount of this order or pay via paypal (click here). If not, your registration certificates will not be mailed until you call in to the office with a credit card number or your check arrives. 5. All headings in red are required. 6. You will notice we ask for your member code if known. We will try to move to requiring the number. It is unique. There is little distinction between John Doe Suffolks and John Doe Family Suffolks, which may include Grandpa and all the kids.
